Durham County Basketball Extra player Registration Form
This from can only be used to register individual player who are an addition to a previously registered team
If the player requires registration to England Basketball then enclose the required amount. Cheques should be made payable to Durham County Basketball Association (D.C.B.A.)
If the player is already England Basketball registered then no fee is required, quote the EB registration number on this form, acceptance may be declined without it.
Please note that 7 days notice from receipt of this form is required before player is eligible to play. It is the clubs responsibility to ensure the 7 day notice period is complete.
